Application Requirements:

1. Completed SURF Application: It is highly recommended to save the application as a PDF on your computer before filling it out.  Applications may also be printed OR attached to an email and mailed to the one of the addresses below.

2. Sealed official transcript:  Include transcripts from all universities if more than one was attended. 

3. Two letters of recommendation:  Letters of recommendation should come from faculty who can evaluate academic and/or research performance.  Letters from family or friends are highly discouraged.
